Why You Feel Good After Physio… But Worse the Next Day

You walk out of your physio appointment feeling like a new person. The pain has eased. Your movement feels freer. You think, finally, we are getting somewhere.

Then you wake up the next morning and it is all back.

If this has happened to you, you are not imagining it. You are not broken. And you are definitely not alone. This cycle is one of the most common and most frustrating experiences people have in physiotherapy, and the reason it happens is worth understanding.

Short-Term Relief vs Long-Term Change

There is an important distinction that does not get talked about enough in clinic: feeling better and actually getting better are not always the same thing.

Short-term relief is real. When pain settles after a treatment, that is genuine. Your nervous system has calmed down, muscle tension has reduced, and your body feels like it has caught a break. That relief matters, and it is not nothing.

But relief is not the same as change. For something to genuinely improve, the underlying tissue, whether that is a tendon, a muscle, a joint, or a movement pattern, needs to adapt. And adaptation takes load, repetition, and time. It does not happen on a treatment table in forty-five minutes.

Think of it like a sore muscle after a hard run. Applying heat and having a massage will make it feel better in the short term. But the muscle does not actually get stronger from the massage. It gets stronger from running again, progressively and consistently, over weeks and months.

Physio is no different.

 

The Role of Passive Treatment

Passive treatment means anything done to you rather than by you. Massage, joint mobilisation, dry needling, heat, ultrasound. These tools all have legitimate uses and can genuinely help in the right context.

The problem is not the tools themselves. The problem is when passive treatment becomes the whole plan.

If every session is built around making you feel better in the room, and you leave without a clear plan for what you are doing between appointments, the program is working on your symptoms and not on your capacity. Symptoms are the output. Capacity is the input. Treating only the output is a bit like clearing the warning light on your car dashboard without looking under the bonnet. The light goes off, but nothing has actually changed.

Passive treatment can absolutely be a bridge, something to settle things down enough that you can start loading. But the loading has to come. Without it, you are just resetting the same system over and over and wondering why nothing sticks.

Why Symptoms Rebound

Your body is remarkably good at returning to its baseline. If that baseline includes a sensitised nervous system, an underloaded tendon, or a movement pattern that puts excess stress on a particular structure, it will keep defaulting back to that state once the treatment effect wears off.

This is not a failure of your body. It is actually your body doing exactly what it is designed to do, maintaining a stable state. The issue is that the stable state it is maintaining is the one that produces pain.

Changing that default requires changing the inputs. More specifically, it requires gradually increasing what your body is asked to do so that it adapts to a new, higher-capacity baseline. That process takes weeks, not hours.

When symptoms rebound after each session, it is usually a signal that the treatment is managing the current state rather than building a new one.

Load Tolerance Mismatch

Here is a concept that explains a lot of rehab frustration: load tolerance.

Every tissue in your body has a tolerance, the amount of stress it can handle before it gets aggravated. When the load you put through a tissue exceeds its tolerance, it protests. Pain, stiffness, and swelling are all ways your body communicates that the demand outstripped the capacity.

The boom-bust cycle most people experience in rehab goes like this. You feel good after treatment, so you do more. Doing more exceeds your current tolerance. You feel worse. You pull back. You feel better again. You do more. Repeat indefinitely.

The fix is not to do less permanently. The fix is to build tolerance gradually so that the gap between what you can handle and what you are actually doing closes over time. That is the entire point of progressive rehabilitation.

Fixing the Boom-Bust Cycle

Getting out of this cycle requires three things.

First, consistency over intensity. Doing a moderate amount regularly beats doing a lot occasionally every single time. Your tissues adapt to repeated, manageable stress. They get aggravated by irregular spikes.

Second, a program that actually progresses. If you are doing the same exercises at the same load week after week, you are maintaining, not building. Progression is what drives adaptation. Each phase of your rehab should be slightly more demanding than the last.

Third, understanding the difference between pain that signals damage and pain that signals adaptation. Some discomfort during rehab is normal and expected. A three to four out of ten during exercise that settles within twenty-four hours is generally fine. Sharp, worsening, or prolonged pain is not. Knowing the difference means you can keep pushing forward rather than stopping every time you feel anything.

What Sustainable Progress Looks Like

Sustainable progress is not a straight line. There will be good days and harder days, and that is normal. But the overall trajectory should be moving in one direction over weeks and months.

It looks like being able to do more than you could six weeks ago. Carrying more. Moving faster. Training harder. Sitting through a full work day without the same level of discomfort. Returning to the activity you had to stop.

It does not look like feeling great on Tuesday and back to square one by Thursday. If that is your experience right now, it is worth asking whether your program is building capacity or just managing symptoms.

You deserve more than temporary relief. You deserve a plan that actually changes something.
