1. Introduction

Welcome to the website of Human Performance Lab Physiotherapy (“we”, “us”, “our”). By accessing or using this website and/or booking our physiotherapy services, you agree to be bound by these Terms & Conditions.

2. Services

Human Performance Lab Physiotherapy provides physiotherapy and related services including, but not limited to:

  • Initial and follow-up physiotherapy consultations

  • Sports injury rehabilitation and performance programming

  • Pre- and post-operative rehabilitation

  • Performance testing and screening

  • Team and club physiotherapy services

  • Educational content, programs, and workshops

All services are delivered in accordance with Australian professional standards and the regulations set out by the Australian Health Practitioner Regulation Agency (AHPRA) and relevant professional bodies.

3. Appointments, Cancellations & Rescheduling

3.1 Booking Appointments

Appointments can be made through our online booking system, by phone, or in person. By making an appointment, you confirm that the information you provide is accurate and up to date.

3.2 Cancellation Policy

We require at least 24 hours’ notice for appointment cancellations or rescheduling. Cancellations made with less than 24 hours’ notice may incur a late cancellation fee, which is at our discretion.

3.3 No-Shows

If you do not attend your scheduled appointment without prior notice, you may be charged the full consultation fee. Repeated no-shows may result in the suspension of future bookings.

3.4 Clinic Right to Reschedule

We reserve the right to reschedule or cancel appointments due to unforeseen circumstances (e.g. staff illness or emergencies). If this occurs, we will make every reasonable effort to notify you and reschedule at a convenient time.

4. Fees, Payments & Rebates

4.1 Fees

All service fees are clearly displayed on our website and may be updated from time to time. Prices are in Australian Dollars (AUD) and include GST where applicable.

4.2 Payment Terms

Payment is required at the time of consultation unless otherwise agreed in advance. We accept major payment methods including EFTPOS, credit card, HICAPS, and approved digital payment platforms.

4.3 Private Health Rebates

If you hold appropriate private health insurance cover, rebates may be available. The amount of any rebate depends on your insurer and policy. Please check with your health fund directly.

4.4 Medicare & Third-Party Funding

Where applicable, we process Medicare and other funding programs (e.g. EPC, NDIS, WorkCover, DVA) according to relevant regulations. You are responsible for providing correct referral documentation prior to your appointment.

5. Your Responsibilities

By booking and attending appointments, you agree to:

  • Provide accurate personal and health information.

  • Inform your physiotherapist of any changes to your health status.

  • Follow advice, exercises, and programs as recommended to achieve optimal results.

  • Notify us promptly of any concerns or issues relating to your treatment.

  • Behave respectfully toward staff and other clients.

Failure to do so may impact your treatment outcomes and your ability to continue receiving services.

6. Medical Disclaimer

All content on this website (including blogs, programs, or social media posts) is for general informational purposes only and does not replace individual physiotherapy or medical advice.
